Job Description

Job Duties Coordinates overall development and implementation of client-based habilitative programming (e.g., individual habilitation plan, individual program plan, individual behavior guidelines) and acts as chairperson of inter-disciplinary team meetings to coordinate comprehensive delivery of services (speech, medical, pharmacy, social services, psychology, psychiatry, vocational, educational) and model appropriate behavior/intervention to an assigned caseload of 8‑30 clients. Supervises assigned staff in the delivery of services, trains staff in effective delivery techniques, ensures habilitative services and documentation meet Medicaid ICF/IID requirements, and maintains accurate records of program implementation. Conducts periodic rounds of cottages/modules/units/buildings or other active treatment locations to ensure proper implementation of programming and to monitor staff in proper documentation, appropriate client interactions, and compliance with Medicaid ICF/IID regulations and standards; serves as chairperson of team meetings to review, analyze, and modify 30/90 day and annual reviews of client habilitation plans; facilitates discussion, develops goals and objectives for clients, prioritizes goals, proofreads minutes of meetings, and writes individual programs, changes, or modifications of clients’ programs on an ongoing basis and corrects deficiencies. Participates in mock surveys of facilities and recommends proposed policies and procedures. Coordinates client home visits, transfers (inter/intra‑facility), and discharges to ensure efficiency; acts as relief residential care supervisor or assists therapeutic program workers in training clients; attends meetings, training, workshops and seminars; serves on committees and task forces; acts as liaison with client families, guardians, case management staff or community‑based staff; prepares reports, forms, and correspondence related to clients and staff assigned; assists with coordination of youth admissions and releases.
